Any person, organisation or group (including The Dean Team) who intends to raise funds for Parkinson's NSW accepts the following:

My participation and the activities I run is solely at my discretion, and I have undertaken my participation freely, voluntarily and absolutely at my own risk and with a full appreciation of the nature and extent of all risks involved in the activities I may organise.

I agree to ensure that any fundraising I undertake will follow the Parkinson's NSW Community Fundraising Guidelines and comply with any relevant Federal, State or local legislation.

Please remember that your fundraising activity will not be a Parkinson's NSW fundraising activity. It will be a fundraising activity to raise funds for Parkinson's NSW. 

The Authorised Fundraiser agrees to release Parkinson's NSW from any claims associated with the activity and indemnify Parkinson's NSW for any liability or costs that may arise regarding damage, loss, or injury occurring to any person associated with the activity caused by a breach of responsibilities or negligence.

My participation and the activities I run are solely at my discretion. I have undertaken my participation freely, voluntarily and absolutely at my own risk and with a full appreciation of the nature and extent of all risks involved in the activities I may organise.

I confirm that I am over the age of eighteen (18) years or have my parent or official guardian’s permission to participate.  

I also give permission for the free use by Parkinson's NSW of my name, image and voice in any broadcast or any other account on social media, the website or any media of Parkinson's NSW.

We may use your personal information for purposes of supporting your participation (including communicating with you and sending you information about the campaign through various platforms, including email, SMS, Facebook, Instagram, Twitter and other social media platforms), re-market to you after the campaign, for other purposes explained in our Privacy Policy or as the law permits. 

You have the opportunity to unsubscribe or modify your preferences for how and when you are contacted at any time via the respective platform or by contacting Parkinson's NSW.
